Your first visit to Evergreen Dental involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You can also save some time by printing out and completing the patient forms in advance of your appointment.
 

If you have DENTAL insurance, please bring your DENTAL insurance card/information to your first appointment.  Without your vital DENTAL insurance information, we are unable to process dental insurance claims.  Thank you.

What to Expect

Being well-prepared for your appointment will ensure that Dr. Lau and Dr. Merry have all of the necessary information to provide the best possible care for you. It may also help relieve any unnecessary anxiety you may feel.  If you have not yet completed our new patient forms, please arrive 5-10 minutes prior to your appointment time to complete them in office.  Otherwise, you’re welcome to download the registration forms.   Also, take some time to review our team page.
